This form is a tool between the requestor and the missions team from Restoration Church Camano Island (RCC) to determine alignment for support.  This document should not be considered a commitment nor endorsement of the missionary or activities.  Confirmation of support will be required by the RCC missions team in writing no less than one month after receipt of this completed request.
